The fault

Leaving the bar in front of the body and calling it locked out.

Why it happens

Most lifters who leave the bar out front do not have a strength problem, they have a definition problem. They have never been shown where overhead actually is.

The fix

A short explanation of where the bar needs to sit relative to the body. Once you can see it, you can find it.
